This article aims to understand the aspects of the spread of Covid-19 in the Tocantina Region of the state of Maranhão based on historically existing inequalities in the supply of health services. The analyzed period comprises one year of a pandemic in the state. Data from the spatialization of Intensive Care Units (ICU), hospital beds, mechanical respirators/ventilators and six medical specialties are considered, as well as information about viral evolution in the 22 municipalities that make up the region. The main data sources are the Department of Informatics, which belongs to the Unified Health System (Datasus) and the State Department of Health of Maranhão. The aspects studied suggest that the concentration of medical specialties and medical-hospital equipment in the main urban center of the Tocantina Region, the city of Imperatriz, is configured as an expressive bottleneck in facing the Covid-19 pandemic, insofar as it triggers flows of people from centers with fewer services in search of medical care, resulting in wide contagion areas, in addition to distortions as to the geographical cases origin.
