COORDENAÇÃO DO CUIDADO ENTRE ATENÇÃO PRIMÁRIA À SAÚDE E ATENÇÃO ESPECIALIZADA NA VISÃO DO PROFISSIONAL MÉDICO: UM ESTUDO TRANSVERSAL

Objetivo: Analisar dimensões da coordenação clínica do cuidado entre médicos da Atenção Primária à Saúde (APS) e Atenção Especializada (AE), considerando fatores laborais, organizacionais, atitudinais e de interação. Método: Estudo transversal com médicos da APS (51) e da AE (73) em um município de grande porte, com utilização do instrumento COORDENA-BR®. Resultados: A maioria dos médicos concordou que não havia troca de informações sobre pacientes em comum (89,5%). A percepção sobre a coerência da atenção foi mais positiva na APS (80,4%). Os especialistas não faziam orientações aos profissionais da APS (74%) que, por sua vez, não os consultavam no caso de dúvidas (84,3%). A maioria dos especialistas não sabia se os usuários eram atendidos na APS para continuidade assistencial (69,9%). Médicos da APS apresentaram maior carga horária no serviço, vínculos trabalhistas precarizados e consideravam o tempo de consulta insuficiente para a coordenação. Os profissionais não se conheciam pessoalmente. Estratégias que promovam comunicação, relacionamento interpessoal e informatização foram consideradas facilitadoras da coordenação. Conclusão: A coordenação entre médicos enfrenta desafios relacionados à falta de comunicação e coerência da atenção. A construção de relações de confiança, prontuários eletrônicos compartilhados e melhoria das condições de trabalho são essenciais para garantir uma abordagem mais integrada.
